[Taxacom] Entry form for the fungal herbarium database

I am looking for the entry form for the UBC fungal herbarium database.

The UBC herbarium has this as a SEARCH form for their database:

https://herbweb.botany.ubc.ca/herbarium/search.php?Database=fungi  

Should we start with something like that for designing the UBC entry form,
or do you have something better?

(P.S. I tried to post this in the HERBARIA list, where it would fit better,
but it did not work. - Apologies!)
