The best documented source would be the annual report Barbara Thiers provided based on records in Index herbariorum.  See http://sweetgum.nybg.org/science/ih/annual-report/

BUT: there are many herbaria that have not updated their records for 20 years or more. The Index is only as good as the information it receives. If you are in charge of a herbarium, please check its record in Index herbariorum (main page is http://sweetgum.nybg.org/science/ih/) and make sure it is current. Records can now be edited online. For information on how to do this, see the "How to update Index herbariorum" http://sweetgum.nybg.org/science/ih/how-to-update-index-herbariorum/).
NOTE: there is now a table asking for more detailed information about a herbarium's holdings than in the past. All too often there is no information in it.  Check that it has been completed for YOUR herbarium (if you are not the top person, encourage the person in charge to see it is done).  
NOT SURE?: Records of individual herbaria can be located by searching by country and/or city. 
ALSO: If you know of a herbarium that is not yet registered (check its country and city), try to persuade the person in charge to do so. It is free and helps us all.  There is a button for registering a new herbarium on the home page.  It takes you to http://sweetgum.nybg.org/science/ih/ihh-create-new/ where there is also a link to more information on how to do it.
