[Taxacom] Page size of BHL generated PDFs

Apologies in advance for doing this all in English units, but it's what we
are stuck with in the U.S.

For the journal I looked at, I think the page height is really about 10"
and had a BHL image height of 2378 pixels. Hence it looks as though the
pages are scanned at about 250 dots per inch (based on the single example I
picked [N=1 science at its finest]).

If we assume that the PDF-constructor somewhere slaps in a resolution of 72
dots per inch (common for screen displays), then that yields the roughly
3.5x enlargement of the resulting document.

I suspect there's a setting in iTextSharp (which BHL is using to do the PDF
generation) that needs to be tweaked to supply the actual dots-per-inch of
the scan, preventing the default 72 dots-per-inch from being imposed on the
document.
