Many thanks to everyone after Valery and Holger who sent me a copy of the
paper.

Apologies that I did not acknowledge your assistance before now but I was
knocked off of my bicycle by a car on Tuesday morning (UK time) have had
not checked my emails until now.  I'm fine, nothing broken just bruising
and pain.
