Basically, I would assume that the journal editor would take Tom Lammers
approach to a reviewer's comments. On the other hand, is there any
reason that you cannot simply add the reference suggested? You mentioned
citing a reference that meant more to you for various reasons; perhaps
the one suggested would mean more to someone else.

The point at which ethics would be involved is if the reviewer making
the suggestion was not familiar with reference you cited AND was a
co-autor of the one suggested - or friends of one of its authors.  To
reiterate, however, take the comment as a suggestion and, if you do not
wish to use it tell the editor why not.
