How about same day, same rank, same author, same publication?

Here is an example from Insecta Diptera Chironomidae that I could
think of immediately (there might be more):

Tanypus nebulosus Meigen, 1804: 21 and 23.

The homonymy was resolved by Meigen's (1818) synonymization of the
latter T. nebulosus (1804: 23) with Tanypus punctatus (Fabricius,
1805), which selected the former T. nebulosus (1804: 21)
as the senior of the two homonyms. The present valid names for the two
species are Macropelopia nebulosa (Meigen) and Natarsia punctata
(Fabricius).
