Peruvian moth id

Hi everyone,
I am looking for a little help from our entomologists out there.  I
recently completed a study of pollination biology of Andean Solanaceae.  I
collected a moth visiting one of my species -- it is just over 1cm long and
has strange structures behind its head that are reminiscent of football
players' shoulder pads.   I have put a bunch of pictures on the web (the
links are below).  Does anyone know the family and genus this animal
belongs to?
