What's in a Name? Clarification

Greetings again. In my previous post, I neglected to fully qualify each of
the botanical-style citations of my example name(s). I should have included
the "subsp." abbreviation, as follows:
Anthias ventralis Randall subsp. hawaiiensis Randall
Pseudanthias ventralis (Randall) Smith subsp. hawaiiensis (Randall) Hoover
Pseudanthias hawaiiensis (Randall) Randall
Also, I have received conflicting information about whether or not one is
supposed to cite the authorship of the non-terminal infrageneric epithets,
or should only include the authorship for the terminal epithet. Comments?
