registration: two questions

I have two questions concerning the registration proposal.
  Suppose an editor is slow about sending in the resistration
forms to the registration committee. She does not do so for
six months or a year, or, worse, she is unaware of the registration
requirements until someone informs her of this five years later.

1) Is there a maximum time limit after which the name is dead
and cannot be registered?

2) From date is priority calculated, the date of publication or
the date of registration?
