My guess is that whether or not a dissertation can contain material that
has already been published is a function of which institution is awarding
the degree.  I know of at least three instances (three different U.S.
universities) in which an already published paper was included as a
"chapter" in a dissertation (perhaps re-written to conform to
dissertation style).

I also think that Lammers has an excellent idea (a "key" to nomenclatural
problems) and that Wuster's suggestion (include such a key with the code)
is a logical follow-up.  Sort of like statistics texts that include a
guide to deciding which test is appropriate for your data/problem.
