Gould, F.W. & R.B. Shaw, 1983. Grass systematics, 2nd ed. Texas A&M
University, College Station. ISBN 0-89-96-153-0.
It contains the items you asked for as well as a nice review of U.S.A.
grasses. I don't know whether new insights in grass classification were
updated in the 2nd edition.
